E. All of the above. Anything that stimulates circulation to the extremities can lead to DCS symptoms. Avoid pulling the anchor, alcohol, etc. Flying contributes to the problem by further lowering the ambient pressure and fostering bubble growth. Limit elevation above sea level as much as possible for 12 hours following a no-decompression dives and for at least 24 hours following dives with required decompression.
